What is hand rejuvenation?

A youthful appearance is often judged upon the condition of not only the face and neck, but also of the hands. Over time, hands can begin to become pigmented, lose fat, and become wrinkled. Hand rejuvenation can help to reverse these problems. This can be done with a combination of lasers including the fractional carbon dioxide laser, plasma skin regeneration and injectables.

How can I prevent my hands from ageing?

There is no real method of preventing hands from ageing other than the use of a good sunscreen. It is important to place sunscreen on the back of the hands as well as the face and neck.

How do I treat skin pigmentation/sunspots on the backs of my hands?

Skin pigmentation on the hands, in the majority of cases, is due to sun exposure. With increasing levels of UV light, the hands develop discrete brown spots that can make the hands appear older.

Hand pigmentation can be treated in a number of ways;

Gemini laser -  The Gemini laser used on the 532nm setting is attracted to the clumps of melanin in the sunspots. Because it is not attracted as much to normal skin as to sunspots/pigmentation, it selectively targets the sunspots/pigmentation. The laser energy is converted to heat energy in the pigmentation, and this disrupts the pigmentation and allows the bodys immune system to carry the pigmentation away. Usually 1 to 5 treatments are required to achieve optimal results. The pigmentation appears darker after the treatment, and then proceeds to flake off over the next few weeks. Each treatment should achieve progressive lightening of the sunspots. Skin over the back of the hands is usually thinner than on the face, and generally lower settings are used on the lasers.

Medlite q-switched laser - The Medlite laser operates on similar principles to the Gemini laser, however, the pulse duration of the laser is much quicker. Hence, it not only heats the pigmentation, but also shatters it. This means that fewer treatments are required, usually 1 to 3, however, the risk is slightly higher.

Ruby laser - The ruby laser can penetrate even deeper than the Medlite and Gemini laser, and can remove any stubborn pigmentation that is present. It is slightly more aggressive than the Gemini and Medlite laser in regards to its side effect profile in this instance, and may require a longer recovery.

Fractional carbon dioxide laser resurfacing of the hands -  Unlike traditional carbon dioxide lasers, which have a high risk of scarring when used on off-face areas like the hands, fractional carbon dioxide lasers are able to remove pigmentation from hands as well as improve skin texture.

Rhytec portrait plasma skin regeneration - Plasma skin regeneration causes generalised heating of the superficial and deep layers of the skin. This has the advantage of improving wrinkles and tightening skin also. It is not a laser, and hence, it does not selectively target skin pigmentation. It is therefore not the treatment of choice if pigmentation is the only problem.

How do I treat the wrinkles on the backs of my hands?

Fractional carbon dioxide laser and plasma skin regeneration can be used at low energy levels to rejuvenate the backs of the hands. These machines are used to resurface skin and allow the skin to regenerate to become smoother and less wrinkly. 

Fractional carbon dioxide lasers ablate a percentage of the surface of the skin, leaving some intact and untreated bridges of skin in between the treated areas. This allows for faster healing and less risk of scarring than traditional unfractionated carbon dioxide lasers.

Plasma energy non-specifically heats the skin, which stimulates collagen production. This in turn reduces wrinkles and tightens skin. As stated previously, this also helps to even out pigmentation. Plasma skin regeneration is therefore the treatment of choice if wrinkles and pigmentation are to be treated.

Generally, 3 treatments spaced 3 weeks apart are required (at low energy levels).

How can I treat fat loss on the back of the hands?

Fat loss on the back of the hands can lead to excessive show of tendons/bone, and make hands appear aged. 

Fat transfer can be used to transfer fat from another part of the body, via liposuction, to be placed in the hands. This is a good procedure that usually lasts for some years.

Injectable dermal fillers can be placed into the hands to restore volume loss here. This helps to reduce the appearance of tendons and the bones of the hand by plumping the areas around these structures.

Usually one to two syringes of Filler type J/JU+/R/P/Radiesse are required per hand.
